Since 2016, Premiere Care has been providing on-site primary care services to residents of assisted living, independent living, and memory care communities throughout Southwest Florida — built on the belief that older adults deserve care that is timely, consistent, and genuinely attentive to who they are.
Meet Your Partner in Care
Tiffany Schoen, APRN, FNP-C, ENP-C, founded Premiere Care after more than two decades of clinical practice across emergency medicine, geriatrics, chronic disease management, post-operative care, and wound care. Throughout her career, she witnessed the same challenge repeatedly: seniors in residential communities were not receiving the timely, consistent primary care they needed, and the families and staff around them were left to fill the gaps.
Her solution was to change the model entirely. By bringing a board-certified nurse practitioner directly on site, she could provide the responsiveness and continuity that traditional office-based care so often fails to deliver. When she established Premiere Care as an independent practice, every one of her existing patients chose to continue their care with her, a reflection of the trust she had built and the standard she has always held herself to.
Outside of her work, Tiffany finds her greatest joy in time spent with her daughters.
