Premiere Care provides a full continuum of on-site primary care services, from routine wellness visits and chronic disease management to acute evaluations and post-hospitalization follow-up, coordinated closely with facility staff, home health partners, and specialists.
Responsive Support
Facility nursing staff can reach Premiere Care 24-hours a day for urgent clinical needs, and families can expect a same-day response. Our provider's emergency medicine background means acute situations are assessed and managed on site whenever clinically appropriate, reducing unnecessary hospital transfers.
Individualized Care
Every resident receives a care plan built around their specific needs, history, and goals. Routine visits occur monthly at minimum, with additional appointments scheduled as conditions require — covering chronic disease management, medication reconciliation, preventive care, and post-hospitalization recovery.

Premiere Care was founded by Tiffany Schoen, APRN, FNP-C, ENP-C, a board-certified Family and Emergency Nurse Practitioner who has been providing on-site primary care to senior living communities throughout the Ft. Myers region since 2016. With a clinical background spanning emergency medicine, geriatrics, chronic disease management, and more, she built Premiere Care around the belief that every resident deserves a provider who is present, responsive, and deeply invested in their wellbeing.
